New patient forms
We offer both paper and digital new patient forms to make the intake process as convenient as possible for your family. Completing your forms ahead of time means less time in the waiting room and more time for our team to focus on your child from the moment they arrive. Whether you prefer to fill them out digitally before your appointment or bring paper forms with you on the day of your visit, we are happy to accommodate whichever works best for you. At Pediatric Dental Care, we've eliminated that barrier entirely. With a dental anesthesiologist on-site every day of the week, we're able to provide general anesthesia right here in our office, making the process faster, simpler, and far less stressful for your family.

Our dental anesthesiologist is available five days a week, a distinction that makes us truly unique. This means that, once they are evaluated by our dental team, your child can receive the care they need without being referred out to a hospital or waiting months for an opening at an outside facility.
